Is the social gradient of female obesity in lower income settings reversing and why? An investigation into the association between wealth, education and obesity

Female levels of obesity in low- and middle-income countries (LMICs) are rising. Obesity prevalence has been the greatest in high socio-economic status (SES) groups, but recent evidence suggests a changing pattern with the prevalence of obesity rising rapidly among lower SES groups and exceeding the former - a reversal of the social gradient of obesity. However, inconsistencies in the gradient by SES indicator have put the reversal into question. Using nationally representative surveys, the thesis: 1) examines the time variation in the SES-obesity association splitting SES into two components; and 2) tests the hypothesis that education protects against the obesogenic effects of improved material circ*mstances through cognitive skills and their influence on media exposure and dietary behaviour. Egypt (~40% female obesity prevalence) is investigated using Demographic and Health Surveys data. Changes in the SES-obesity association over the period 1992-2008 are examined including the se...

Use of different subjective health indicators to assess health inequalities in an urban immigrant population in north-western Italy: a cross-sectional study

Background Despite the steady growth of the immigrant population in Italy, data on the health status of immigrants are scarce. Our main goals were to measure Health-Related Quality of Life (HRQoL), Self-Rated Health (SRH) and morbidity among immigrants in Genoa. We aimed to assess the relative contribution of some social, structural and behavioral determinants to “within-group” health disparities. Methods We enrolled 502 subjects by means of snowball sampling. The SF-12 questionnaire, integrated with socio-demographic and health-related items, was used. Multivariate logistic and Poisson regression models were applied in order to identify characteristics associated with poor SRH, lower SF-12 scores and prevalence of self-reported morbidities. Results Subjects showed relatively moderate levels of HRQoL (median physical and mental scores of 51.6 and 47.3, respectively) and about 15% of them rated their health as fair or poor. Lower scores in the physical dimension of HRQoL were associa...

Biocultural determinants of overweight and obesity in the context of nutrition transition in Senegal: a holistic anthropological approach

Senegal is experiencing a rising obesity epidemic, due to the nutrition transition occurring in most African countries, and driven by sedentary behaviour and high-calorie dietary intake. In addition, the anthropological local drivers of the social valorization of processed high-calorie food and large body sizes could expose the population to obesity risk. This study aimed to determine the impact of these biocultural factors on the nutritional status of Senegalese adults. A mixed methods approach was used, including qualitative and quantitative studies. Between 2011 and 2013, fourteen focus group discussions (n=84) and a cross-sectional quantitative survey (n=313 women; n=284 men) of adults in three different socio-ecological areas of Senegal (rural: n=204; suburban: n=206; urban: n=187) were conducted. Dietary intake (Dietary Diversity Scores), physical activity (International Physical Activity Questionnaire), body weight norms (Body Size Scale), weight and health statuses (anthropo...

Overweight/obesity rates in children of immigrants from low-and-middle-income-countries not only eclipse rates in their origin countries, they also exceed rates in their host nation. My mixed-method research seek to understand this conundrum, to inform a more effective, and equitable prevention agenda. Using representative data from Birth cohort of the "Longitudinal Study of Australian Children", I quantified the prevalence and predictors of overweight/obesity in Australian-born children by maternal immigrant status. For the qualitative component, I interviewed immigrant mothers and their children, to understand how conflicts of culture and acculturation in the host countries contribute to greater risk in these children. My first paper confirmed that not only do the children of mothers from low-and-middle-income-countries matched the overweight/obesity rates of children of Australian-born mothers, they superseded them at most ages. The difference was significantly higher i...
